چکیده

Objective: To investigate the clinical effect of Dynesys dynamic fixation for double-segmental lumbar disc herniation and the degeneration of adjacent segment. Methods: 52 patients, who received Dynesys dynamic fixation for double-segmental lumbar disc herniation in our hospital from Mar 2009 to Mar 2012, were enrolled as research subjects; among them, 34 were male and 18 were female with an average age of (44.3±3.6) years old (19-67 years old). Oswestry dysfunction index (ODI) and visual analogue scale (VAS) was evaluated before surgery and during the outpatient follow-up in two years after surgery. X-ray and MRI on lumbar spine were done to evaluate the intervertebral height of adjacent segments and signal intensity of nucleus pulposus, respectively; degeneration was determined as 10% decrease in intervertebral height and one level degrade in signal intensity of nucleus pulposus. Logistics regression analysis was used to explore the risk factors of adjacent segment degeneration (ASD). Results: Two-year follow-up showed that all patients had satisfactory recovery in nerve function and alleviation in pain; ODI was significantly lower than that before operation (21.4±3.0 VS. 66.3±7.9, p=0.001), and VAS decreased significantly by comparing with the pre-operation (2.0±0.1 VS. 6.4±0.5, p < 0.001). There were no significant changes in the intervertabral height nor ROM of adjacent segments (P > 0.05). 9 patients (17.3%) had adjacent segment degeneration showed by imaging; by regression analysis, we found that BMI > 30 kg/m2, age > 50 years old were the risk factors for adjacent segment degeneration. Conclusion: Dynesys dynamic fixation in treating double-segmental lumbar disc herniation could achieve satisfactory effects in nerve function recovery and pain alleviation, obesity and old age are the risk factors for adjacent segment degeneration.

Medium-term effects of Dynesys dynamic stabilization versus posterior lumbar interbody fusion for treatment of multisegmental lumbar degenerative disease

Objective To compare the medium-term clinical and radiographic outcomes of Dynesys dynamic stabilization and posterior lumbar interbody fusion (PLIF) for treatment of multisegmental lumbar degenerative disease. Methods Fifty-seven patients with multisegmental lumbar degenerative disease underwent Dynesys stabilization (n = 26) or PLIF (n = 31) from December 2008 to February 2010. The mean follo...

Screw loosening and Migration after Dynesys Implantation

Due to the problems associated with rigid fixation, the concept of dynamic stabilization was introduced. Dynesys is one of the pedicle screw-based dynamic stabilization systems. In spite of the dynamic nature of Dynesys, there are concerns about the rigidity of the Dynesys system. We present a case of vertebral body split fracture after Dynesys implantation in a 74-year-old woman. She had under...

Finite element simulation and clinical follow-up of lumbar spine biomechanics with dynamic fixations

Arthrodesis is a recommended treatment in advanced stages of degenerative disc disease. Despite dynamic fixations were designed to prevent abnormal motions with better physiological load transmission, improving lumbar pain and reducing stress on adjacent segments, contradictory results have been obtained. This study was designed to compare differences in the biomechanical behaviour between the ...

EARLY AND DELAYED RESULTS OF LUMBAR DISC SURGERY IN 314 PATIENTS

In a retrospective study the clinical presentations, neurological findings, contrast studies, operative findings, and the follow-up results of 314 patients with disc herniation were studied over a 10-year period. This disease was most frequent between the ages of 30 to 40 and mostly in males. Low back pain with or without radicular pain was the most frequent symptom, involving the left leg ...

Comparison the Recurrence Rate of Intervertebral Disc Herniation in Two Surgical Methods of Hemi laminectomy and Partial Laminectomy

Background & Aims: Between 5% and 15% of patients with low back pain suffer from lumbar disc herniation, so intervertebral disc disease is one of the most common causes of low back pain. Surgical intervention for lumbar disc herniation is recommended in patients with severe symptoms and has hopeful results. Although surgery can reduce pain and improve physical function in the short term, its lo...
